Record and Investigate a Work-Related Incident

How to record a work-related health and safety incident in preeco | nachhaltigkeit, start the investigation and close it with a root cause and corrective actions – the data basis for the ESRS S1-14 metrics.

A workplace accident is recorded in preeco | nachhaltigkeit as an “H&S Incident” and then runs through an investigation workflow with the statuses “Open”“Investigating”“Closed”. Only a documented closure with a root cause and corrective actions makes the ESRS S1-14 metrics audit-proof.

Prerequisites

  • A locked CSRD reporting boundary with included organizations exists for the selected reporting period – it determines which organizations are selectable.
  • The H&S data for the period is not yet marked as complete. Otherwise the “Add Incident” button is hidden.
  • You are signed in to your team and the relevant reporting period is selected in the header bar.

Record the incident

  1. Open “Own Workforce > Health & Safety”.
  2. In the “H&S Incidents” section, click “Add Incident”. The “Add H&S Incident” dialog opens in a side panel.
  3. In the “Location” section, select the affected “Organization” (mandatory field). The “Facility” is optional; the default is Organization-level (no specific facility).
  4. In the “Incident details” section, enter the “Incident Date”, the “Incident Type” (Injury, Ill Health, Fatality or High-Consequence Injury), the “Severity” (Minor, Moderate, Severe or Fatal) and the “Description”. Do not enter names or personal data in the description – the dialog states this explicitly with “Do not include worker names or personal identifiers.”
  5. In the “Classification” section, set the “Worker Type” (Employee or Non-Employee), check the “Recordable” toggle that is enabled by default, and enter the days lost in calendar days under “Lost Days”.
  6. If you select Fatality as the “Incident Type”, the additional mandatory field “Fatality Cause” appears with the options Death from work-related injury and Death from work-related illness. “Severity” is then fixed to Fatal and “Recordable” can no longer be deselected.
  7. Click “Create”. Use “Cancel” to discard the entries.

The incident appears in the “H&S Incidents” table with an automatically assigned reference number (for example INC-000001) and the investigation status “Open”.

Start and close the investigation

  1. Open the incident detail page, open the “Actions” menu at the top right and select “Start Investigation”. The “Investigation Status” changes to “Investigating”.
  2. Once your enquiries are complete, select “Close Investigation” from the “Actions” menu. The dialog of the same name opens in a side panel.
  3. In the “Investigation Closure” section, complete the mandatory fields “Root Cause”, “Corrective Actions” and “Investigation Findings Summary”. “Closure Notes” are optional.
  4. Click “Close Investigation”. The status changes to “Closed” and the information appears in the “Investigation” section of the detail page.
  5. If new findings emerge, select “Actions > Reopen Investigation” and enter the mandatory “Reason for Reopening” (3 to 100 characters). The original closure data is retained and stored read-only.

What happens next

  • The metrics in the “Key Performance Indicators” section (TRIR, LTIFR, HCIR, Recordable Incidents, Lost Days) are recalculated immediately, provided a coverage data record with working hours exists.
  • The investigation status appears in the “Investigation” column of the incident table and feeds into the completeness check of the period.
  • Every status change is logged in the activity history.

Practical tip: close all investigations relating to fatalities before you close the period – that check blocks the period close. The completeness check additionally expects a closure rate of at least 90% across all investigations.
